Payment Responsibility

Epoche Medical is a cash-based medical practice. Payment for all services is the patient’s responsibility and is due at the time services are rendered, unless prior arrangements have been made. A valid credit card is required to be kept on file through our secure electronic medical record system and may be charged for services, fees, or outstanding balances in accordance with this policy. Epoche Medical does not participate with insurance plans. A superbill may be provided upon request for potential out-of-network reimbursement; however, reimbursement is not guaranteed.

No Refund Policy

All payments for services are non-refundable once services have been provided. This includes, but is not limited to, medical consultations, time-based visits, counseling, care coordination, treatment planning, review and interpretation of medical records, laboratory or diagnostic results, and in-office services.  All sales of supplements, vitamins, nutraceuticals, and test kits are final and non-refundable, whether opened, used, or unused.  Payment is for professional time, medical expertise, and services rendered, and is not contingent upon achieving a specific outcome. Individual responses to medical care vary, and outcomes cannot be guaranteed.

Pre-Service Deposits & Outstanding Balances

Epoche Medical requires pre-payment deposits, or full payment prior to scheduling future appointments in cases of prior missed appointments, late cancellations, or outstanding balances. Failure to resolve outstanding balances may result in limitations on scheduling or discontinuation of services, consistent with applicable laws and ethical standards.

Cancellation & Rescheduling Policy

Cancellations or rescheduling requests must be made at least 24 hours in advance.  Cancellations or rescheduling requests made within 24 hours of the scheduled appointment will result in a late cancellation fee of $100, which will be charged to the credit card on file. Failure to attend a scheduled appointment without prior notice will be considered a no-show and will also result in a $100 fee.

In the event an appointment secured by a pre-payment deposit is cancelled or rescheduled, Epoche Medical reserves the right to retain a reasonable administrative and processing fee from the deposit to cover scheduling, administrative, and processing costs incurred. Any remaining balance, if applicable, will be handled in accordance with Epoche Medical’s Financial Policy.

Late Arrival Policy

Patients are expected to arrive on time for their appointments. Arrival more than 15 minutes after the scheduled start time may result in the appointment being rescheduled and documented as a missed appointment, at the provider’s discretion.

Repeated Late Cancellations or No-Shows

Repeated late cancellations or no-shows, defined as more than two occurrences within a 12-month period, may result in additional scheduling restrictions, including the requirement of pre-payment or deposits to hold future appointment times.

Extended Visit Policy

Visits are scheduled for a defined duration. If an appointment extends beyond the scheduled time due to patient needs or complexity, additional time may be billed in increments consistent with clinic policy. Care coordination, phone calls, portal messages, or administrative services exceeding brief communication time may be billed separately.

Epoche Medical provides integrative and functional medical services focused on comprehensive evaluation and individualized care. These services are not emergency care and do not replace the role of a primary care provider. Medical recommendations are made based on clinical judgment, patient history, and available information. Disagreement with a provider’s assessment or recommended plan does not constitute non-delivery of services.
